Sick Notes

The Doctor is able to produce a Med 3 Fit Note after you have been absent from work for more than 7 days.  Prior to this you will need to complete a Self- Certification (available from your employer).  You will be asked to book an appointment  to discuss this request with your Doctor unless you have recently been discharged from hospital.

With prior agreement with your Doctor you may be able to request a continuation of a Med 3 Fit note without the need to see the Doctor.
