We accept patients who are living within our Practice boundaries. If you are unsure whether you reside within our boundaries, please check using our online tool below.
You will need to complete and return all of the registration forms within the registration pack. Please be careful to complete all of the relevant details, not doing so may delay your registration.

Named GP
All patients at Hibaldstow Medical Practice have a named, accountable GP who is responsible for coordinating their care.
If you would like to know who your named GP is, please contact the practice. If you have a preference, we will make reasonable efforts to accommodate your request.
Access to Records
We hold copies of your patient health record electronically and in paper format. When you register with the practice and your registration documents have been processed, your Electronic Record can reach us within minutes.
Access Your Records Online – NHS APP
It is now possible to view your medical record online – using the NHS app. Being able to see your record online might help you to manage your medical conditions. It also means that you can even access it from anywhere in the world should you require medical treatment on holiday.
